Transitional pathways

Becoming an adult is a big change. We help make it easier.

Best Lives supports young adults leaving children’s services to find new opportunities. We also provide housing for adults with learning disabilities in crisis who need emergency accommodation.

We are here to help make the move a positive one.

The transition journey

Moving from children’s services to adult care involves coordination and planning. Best Lives guides families through the process. The pathway is broken down into three key stages.

1.

Early planning (age 16-17)

Preparation starts early to ensure no one feels rushed. Conversations about adult services begin at age 16, with specific goals added to the EHCP. Best Lives joins planning meetings with social workers and families during this time to help shape the future support plan.

2.

Assessment and agreements

Before the move, families request a care needs assessment from the local authority. Once a support plan is agreed, funding is arranged via direct payments or as a commissioned service. This is also the time to address legal changes, as families may need a lasting power of attorney to assist with decisions after age 18.

3.

The move (age 18+)

To make the transition stress-free, Best Lives focuses on building relationships first. The team visits the young person at school and offers taster days or half-term programmes. This phased approach builds confidence and friendships, ensuring the individual feels safe and ready before their official start date.

Crisis intervention

Unexpected challenges can place adults with learning disabilities at risk of losing their homes. The Best Lives crisis intervention service, Higher Road, offers support to help individuals regain stability.

This service provides safe, short-term accommodation built on compassion and understanding. It serves as a kinder alternative to hospital admissions or secure settings. The team focuses on developing daily living skills, helping individuals navigate difficult times with dignity.
